 Things have changed. We're getting more and more requests to transport larger patients every day and, with our traditional ambulances it has become very difficult to move these patients safely. It's also unsafe for our crews.

 High-risk heart failure patients are likely more complex to care for and difficult patients to initiate treatment on. I don't think physicians are giving up on their patients. But they may be struggling with some complex cases.

 This isn't a traditional medical drama. I've always said from the beginning, we're a relationship show with surgery. It's never about the patients. It's about how the doctors feel about the patients.

 Laparoscopy requires a different skill set than open surgery. Surgeons typically view patients from the outside in. When a laparoscopic camera is inserted, they see patients from the inside out. Hand/eye coordination is difficult to master. Practice on a simulator would allow surgeons to perfect their technique with no risk to patients.

 Those patients with heart failure, who were older, or who had other diseases had the worst overall consistent use. There are still doctors who are reluctant to prescribe these drugs to their sickest patients, maybe not realizing that potential side effects are far outweighed by the benefits. This represents a gap in our understanding – why is it that health care providers appear to fear treating these patients as aggressively as other patients?

 Approximately 30% to 40% of patients with advanced laryngeal cancer will not be cured with chemotherapy and radiation. The survival rates for such patients have traditionally been poor. That's why these patients should be identified as early as possible. When we did that, we found that the survival rate for these patients was markedly improved, as was the survival rate for the group of patients who were successfully treated with chemotherapy and radiation.

 We had patients spread over about 300m. There were five ambulances and a helicopter.

 These studies show many more patients can benefit from taking these two drugs, ... Even though the FDA expanded the indications for their use earlier this year to include patients with moderate to severe disease, many patients are not using them.

 We're seeing great results. We're still getting some patients who don't get much of a result, but we are seeing 80 or 90 percent of patients with improved near vision and lots of our patients able to read newspaper, magazines, work on the computer, without reading glasses at all.

 We are continually seeing an increase in the number of obese patients presenting to hospitals. These patients are not necessarily surgical patients but present with a variety of medical needs and can be seen throughout a health care facility.

 When you put someone who weighs 700 pounds on that narrow gurney, it can be awkward. Not only does this person not fit, there's a chance he or she could fall. Our job is to get that patient to where they need to be safely and in a dignified manner. Traditional ambulances can't do that.
