 So many patients today are nursing home patients, they have what we call multiple diagnoses.  Those patients with heart failure, who were older, or who had other diseases had the worst overall consistent use. There are still doctors who are reluctant to prescribe these drugs to their sickest patients, maybe not realizing that potential side effects are far outweighed by the benefits. This represents a gap in our understanding – why is it that health care providers appear to fear treating these patients as aggressively as other patients?

 Over the years, our patients have shared some amazing and emotional stories as they've recovered from an injury, illness, or surgery which required some degree of physical rehabilitation. For many of our patients, once they've experienced a disability, they work with unwavering strength and courage to return to the lifestyle of their choosing. Their stories are compelling and emphasize the perseverance of the human spirit. In sharing their experiences, our former patients are reminded of their own strength, and are benefiting those who are now in a similar situation. Reading these stories will hopefully provide inspiration to those who are at the beginning of their own physical rehabilitation process and others who are not sure what physical rehabilitation is all about.

 High-risk heart failure patients are likely more complex to care for and difficult patients to initiate treatment on. I don't think physicians are giving up on their patients. But they may be struggling with some complex cases.

 The Optimizer is an exciting and innovative technology with the potential to treat patients with Class III or Class IV heart failure. We have already begun enrolling patients in the FIX-HF-5 study to evaluate the safety and effectiveness of the Optimizer. We look forward to completing new patient enrollment in order to fully assess the potential of this device to meet the needs of moderate to severe heart failure patients.

 I felt that the biomedical and psychosocial approaches were insufficient to explain the disabilities of my patients and how those impacted their lives. The physical environment was really an etiological factor. If you have a person with a particular type of disability, you can approach it from the standpoint of reducing the impairment using pharmaceutical management or even a surgical procedure. But there's a point at which medicine and surgery don't work anymore.

 There's not a cure available, so patients are often left somewhat frustrated by continuing pain and fatigue. Acupuncture is one of the few things shown to be effective for these symptoms. It may be particularly attractive to patients who are unable to take medications because of intolerable side effects.

 Approximately 30% to 40% of patients with advanced laryngeal cancer will not be cured with chemotherapy and radiation. The survival rates for such patients have traditionally been poor. That's why these patients should be identified as early as possible. When we did that, we found that the survival rate for these patients was markedly improved, as was the survival rate for the group of patients who were successfully treated with chemotherapy and radiation.

 In counseling patients who are considering gastric bypass surgery, this study will help us to identify those patients who can reasonably expect the long-lasting health benefits from significantly reduced blood pressure.

 Based on our research, it appears that increases in numbers of patients at both ends of the acuity spectrum contribute to ED overcrowding. Some increased volume may be due to severely ill patients who can now be treated successfully in the ED, such as those with irregular heartbeat. At the other end of the spectrum, a growing number of less sick patients are now coming to the ED, particularly in areas without after-hours facilities. This is an access problem.
