This study proves that too many Ohioans are suffering and paying more for care because they're not treating serious conditions, such as diabetes, heart disease and asthma. Patients who are informed about their conditions, and who communicate with their doctors, are better prepared to mitigate threats to health and costs for care. By focusing on prevention rather than intervention, and actively involving physicians to get disease under control, Ohioans can improve their lives and the lives of their families.

Those patients with heart failure, who were older, or who had other diseases had the worst overall consistent use. There are still doctors who are reluctant to prescribe these drugs to their sickest patients, maybe not realizing that potential side effects are far outweighed by the benefits. This represents a gap in our understanding – why is it that health care providers appear to fear treating these patients as aggressively as other patients?

Approximately 30 percent to 40 percent of patients with advanced laryngeal cancer will not be cured with chemotherapy and radiation. The survival rates for such patients have traditionally been poor. That's why these patients should be identified as early as possible. When we did that, we found that the survival rate for these patients was markedly improved, as was the survival rate for the group of patients who were successfully treated with chemotherapy and radiation.

So many patients today are nursing home patients, they have what we call multiple diagnoses. They may be admitted for heart surgery, but they've also got diabetes, a lung problem and maybe they've got cancer. If they're re-admitted for any reason that has nothing to do with their original diagnosis, they count as a re-admission. Patients are so incredibly complex.

[The emphasis today is on relatively low-tech screens for signs of incipient chronic disease -- adult-onset diabetes, hypertension and heart disease, cancer and the like. To help doctors track these indicators, the preventive services task force revises its guidelines continuously. Recently, for example, the task force recommended that doctors screen patients for depression. The screen is simple, almost a casual conversation --] How's work? Are you feeling stressed? Are you sleeping? ... Many people who are depressed don't go to a psychiatrist looking for insight. Most are seen in primary-care situations.

Medicare Part D is the single most significant mental health policy initiative to have been undertaken in the last decade. We think it would be extremely helpful to have a sense of how this is actually impacting our patients and practitioners. The only way of systematically assessing the strengths and weaknesses of the new program is a survey that would get a representative sample of the experiences our patients are having.

This is the first and only long-acting medication available for patients, and so I'm thrilled that it is going to become an important treatment option for patients and their doctors.
