With major insurance companies diverting patients from emergency rooms to our clinic we have expectations of exceeding the original number of 30 patients a day.

Patients still know that we have very good emergency rooms and very good physicians and nurses. This report card does not speak to the quality of care patients receive once they are in the ER. This speaks to all of the challenges that all of the emergency rooms in New Mexico have.

All of our patients can present at emergency rooms (when they're injured or sick), but can they get elective care at these facilities without insurance?

Now they tell their patients to go to hospital emergency rooms if they get sick. We hope to make one-on-one visits with area doctors and convince them to refer their patients after hours to be seen.

In fact, on Monday we had a record-breaking day. We saw a third more patients than we normally do between our two emergency rooms. On any normal day, which is pretty busy, we see 200 patients. We saw 302 on Monday.

We found that over half of patients visiting emergency departments were there for less-urgent or non-urgent conditions, things like chronic back pain or a sore throat. Whereas at the other extreme, less than one per cent who visited emergency rooms were most severely ill - people with diagnoses of shock or major trauma.

We are the only free clinic in the area. Between January and November last year, we saw 500 patients. We have 1,700 patients on our books.

We are continually seeing an increase in the number of obese patients presenting to hospitals. These patients are not necessarily surgical patients but present with a variety of medical needs and can be seen throughout a health care facility.

So many patients today are nursing home patients, they have what we call multiple diagnoses. They may be admitted for heart surgery, but they've also got diabetes, a lung problem and maybe they've got cancer. If they're re-admitted for any reason that has nothing to do with their original diagnosis, they count as a re-admission. Patients are so incredibly complex.

We are up 22 percent from 2001, when we had 16,500 patients. We are looking to expand the emergency room, labor and delivery and add a convenience care clinic.

The Fletcher bill...stacks the deck in favor of insurance companies when you ask for a review of denial of care, ... It does not prohibit insurance companies from using financial incentives to penalize your doctor for authorizing care. The insurance companies may like the Fletcher bill, but doctors and patients don't.

We have been working closely with the Central Social Insurance Medical Council, MHLW and some of the country's leading physicians to bring this valuable technology to patients in Japan. We are excited to be expanding this unique technology to another part of the world, where a significant number of patients can benefit.

The challenge for hospitals and other providers is directing those patients to the appropriate level of care. Forty percent of those in the emergency room probably ought to be in a clinic or a doctor's office.

Emergency departments and ER doctors will take it on the chin. And follow-up care would be difficult for those patients to obtain, especially those without insurance.

The heart attacks, the strokes, they will come no matter what's going on. The patient with pneumonia, the patient with an asthma attack will say, 'Maybe I can ride this out at home.'
