 Since we didn't have a lot of our policies, procedures and standards documented, we needed to come up with a basic framework that we could use to get started.

 Once we adopted a framework for developing policies, standards and procedures, we made really good progress. The documentation process really started to fall into place.

 In the first few years it was kind of like the Wild West. Organizations changed the way they documented policies, processes, and procedures, but not necessarily in a logical manner. What's happening now is that organizations better understand the kinds of documentation procedures that work — there are a lot of successful templates out there.

 When policies and procedures are not documented - or when they are inaccessible, incomprehensible, or difficult to use - the consequences for organizations can be very serious. Both safety and security can be compromised.

 There was so much that wasn't right, ... It needed structure. It needed clear policies and procedures. It needed to have a rule book.

 We're delighted it's been resolved satisfactorily. Although we believe we've had strong policies and procedures in place, we're happy to have voluntarily enhanced existing policies and procedures to ultimately safeguard the interests of our customers in Massachusetts.

 Last fall was not about bad policies and procedures. It was about individuals not following the policies and procedures.

 The fact that I have previously documented and expounded on the basic issues presented in the Harvard paper does not invalidate its easily documented truth. It validates me and my work and the incredible bias of the media that has worked so hard to discredit me.  It now seems clear that we didn't know enough or do enough before publication, and if our traditional procedures did not prevent the mistake, then it is time to clarify and strengthen a number of our policies.

 The feeling has been that the height of the walls is sufficient to discourage fans from jumping out of the stands all the way onto the field. Our security has video of the incident. It will be fully reviewed, and a determination will be made on whether changes are needed in our policies and procedures in this area.

 Our basic concern is to let the consumers choose which basic services they're going to be able to afford under a basic insurance policy. Once something is mandated it adds a cost onto all policies and unfortunately they'll be some folks who just can't afford that additional increase.

 What operators should be doing ?- and what the standards bodies working in this area are encouraging them to do ?- is to create a policy architecture : a unified framework that embraces all the layers of policy needed in their organization.

 Our policies and procedures with respect to claims reviews are designed to ensure that insurance premiums remain affordable for our members. It's important that we issue policies based on accurate information filed by the member.

 It was never documented, employees were not made to sign a contract saying that they understand the rules and procedures and now they are.

 When we'd have arguments over who has the best basic process for development, nobody wins. Rather than controlling the basic process framework, we (at IBM) can leverage what the open-source community does. And other software vendors can do the same around our processes.
