 This year's study reflects the financial distress hospitals have encountered in the wake of the Balanced Budget Act of 1997. Despite the difficulties facing them, the 100 top benchmark hospitals have maintained profitability and top clinical performance.

 We are experiencing the same kinds of problems that are no different from those faced by most other hospitals in the country. Hospitals are losing money because we are receiving lower state and federal reimbursements for patient treatments and are having more uninsured and under-insured patients using emergency rooms as their primary care physicians.

 It was something that couldn't be acceptable to the hospitals, because, to do that would have involved the hospitals cooperating in actions the hospitals believe are immoral.

 We're looking at every point in the process, from scheduling to post-discharge care, because our data shows that hospitals who manage their key service lines in this manner are successful in collaborating with their medical staff. That performance-based collaboration is the key to improving all aspects of performance, including reducing implant costs.

 We started with an oncology nurse, working part time out of her home. Then we could provide enough service to build momentum and credibility, and the first nurse could use her time to make contacts with clinics and hospitals.

 I was a school nurse, and I can tell you that I sent many, many letters to parents telling them about problems and recommending that they take their children to a doctor. I know that many families never followed up.

 At this point in our national rollout, we are making our TV/DVD viewing systems and monthly educational DVD magazines available to hospitals that have multiple viewing locations. We have the opportunity to impact thousands of patients a day in each subscribing hospital location. As our overall viewership increases in these hospitals, it gives us a greater impact in terms of scoring higher advertising rates.
