 In terms of quality, 1999 was by far the best year in the history of managed care. But when we look at care for certain populations like people with diabetes or depression, it's obvious that there is still room for improvement.

 Working with Wal-Mart and SAM'S CLUB allows us to directly reach large populations affected by diabetes, many of whom do not know they are at risk for diabetes or possibly even have the disease, ... Diabetes can be managed and even prevented with diabetes education and the right care. That's what this event is all about.

 Let me be blunt. Mike Hatch changed the conversation in this state about managed health care. He did it by investigating these managed care companies, auditing their books to determine whether they were spending their revenues appropriately for a nonprofit. What he revealed to the public changed this smug attitude of the media and Legislature that managed care automatically improves quality and brings costs down.

 What this means is, if someone from another state is looking for the nearest Joslin Diabetes Center for care, they can come to Las Vegas. This is a major benefit to the community because it puts diabetes in the spotlight here, and people will get better care because of that spotlight no matter where they receive treatment.

 What we are trying to achieve is to identify those hospitals that perform well under adverse conditions, such as managed care, as opposed to those that perform well under the old system of cost-plus, or Medicare. We look at the quality of care for patients, and their efficiency in adapting to the health care environment.

 We don't have any objections to managed care. If people want to join long-term managed-care plans, that's fine with us.

 One of the big factors that may account for this finding is access to high-quality medical care. Often communities that have high poverty rates either lack access to good quality care, or people have to travel longer distances to obtain high-quality care.

 We care about what we do. We care in our presentation, in the quality of the ingredients that we use. We try to use the freshest of everything and import some meats from Ireland. We care about what people think and how they like their meals.

 We are proud to bring the world-renowned leader in diabetes care and education to the Las Vegas Valley. Diabetes is a major disease affecting our local community at a rate that exceeds both state and national norms. If not properly treated, it can be a life threatening and debilitating. We look forward to working with the Joslin staff in Boston to ensure that our care meets the standards of this international leader in diabetes treatment, research and education.

 There is no question that managed care is managed cost, and the idea is that you can save a lot of money and make health care costs less if you ration it.

 In view of the rapidly rising rates of obesity and diabetes in America, this good news could not have come at a better time, ... So many of our health problems can be avoided through diet, exercise and making sure we take care of ourselves. By promoting healthy lifestyles, we can improve the quality of life for all Americans, and reduce health care costs dramatically.

 Not everyone will be able to see a diabetes specialist. Most people with diabetes, especially those in smaller communities, are managing their illness in the primary-care setting. The (tested) intervention gives them, and their providers, better tools with which to do this.

 We really believe this is bad health care policy. We're dealing with folks who have multiple health care issues. Having a route manager on a regular basis will go away. Now they won't have someone there. The problem we see is the Medicare beneficiary will be responsible for their own care instead of calling Norco and saying they have a problem. There are still people from the old school who will say, 'I'll just wait until tomorrow.' Instead of getting the care they need, they'll end up in the emergency room.

 Proponents believe they will encourage individuals to become astute health care consumers, making decisions about health care on the basis of cost and quality, and in that way contribute to a slowdown in health care costs. Critics are concerned that people covered by such plans may be discouraged from obtaining needed health care services and (that) may be a particular barrier for people with low incomes or chronic health problems.

 There are thousands of people each day whose lives and the quality of their health care are affected because we do not have national legislation that sets standards and establishes a set of rules that would ensure that they get quality care, ... That's more than frustrating -- that's unacceptable.
