 These first two weeks the system has just been overwhelmed. On January 2 I spent the day helping a mental health patient get her medicine. It required prior authorization and it took me three hours to get that authorization.

 We had one woman whose diabetes medicine wasn't on the formulary for the plan she was in. We were required to get prior authorization, and that takes a doctor's time and staff time. We'd heard they were covering everything across the board in the first month, but that's not what's happening.

 Proclamation 1017 does not give us the power to close down broadcast outfits. Under EO 546 and in the terms and conditions we have with broadcasting firms given authorization (license to operate), we can impose sanctions including suspension of their authorization and cancellation of the authorization. In so doing, we have to follow due process, through public hearings.

 That authorization was based on an intelligence community assessment of a serious and continuing threat to the homeland. The lawfulness of the actual authorization was reviewed by lawyers at the Department of Justice and the White House, and was approved by the attorney general.

 Most of the elapsed time in a credit card authorization is on initiating and establishing the connection to the payment processor; once the connection is established, the authorization can be completed at the wink of an eye.

 Prior authorization is getting to be a bigger problem every day.

 Effective immediately, we will treat all providers that these members see as if they were 'in network' providers, and affected members will not be required to satisfy prior authorization, pre-certification or referral requirements during this time. It is important at this time that those members seek and receive the medical attention they need right away.

 Under Article 2 of the Constitution, as commander in chief, the president has that authority. The president has the authority under the congressional authorization that was passed and clearly stated that, quote, 'The president is authorized to use all necessary and appropriate force.' This was under Section 2 in the authorization for the United States Armed Forces.

 It gives the pharmacist time to clear any hurdles, like obtaining prior authorization, and to make sure the drug is in stock.

 We hope this will rally member states around the authorization process. We want practical improvements in the system.

 My concern is a single concern - can we keep a safety net under patients so they don't end up in a health crisis just because they cannot get medicine or care? I don't think the system is meeting the needs of the uninsured right now. It's overwhelmed.

 There is increasing chatter that when the in-patient payment system comes out in a couple of weeks, that it will be more controversial than we've seen in prior years.
